What is low level laser therapy LLLT?-We have been using low level laser therapy LLLT in our clinic for a couple of years and continue to see great results for a wide range of conditions including back pain, neck pain, shoulder and knee injuries, bursitis, tennis and golfers elbow to name a few. It’s called cold laser because no heat is generated when the laser beam is pulsed. Another name for it is low level laser therapy (LLLT). It was discovered that low level pulsed laser penetrated living tissue to a greater depth than continuous wave laser and produced no damaging heat.

In 1967 it was accidentally discovered that LLLT regrows hair and accelerates wound healing in mice. In 1979, Russian cosmonauts discovered that LLLT produced positive healing effects on human tissue.

Since then much research has taken place to show that low level laser activates the mitochondria of damaged cells to produce increased cellular energy (ATP), decreased inflammation and increased micro-circulation. These effects combine to produce the healing we observe while at the same time producing no adverse side effects.

Multi Radiance Medical launched in the USA in 2004. They combined low level super pulsed laser with an electro-magnetic field and infrared energy to have a more powerful effect. This is the type of laser therapy we use in our clinic. Strong levels of evidence exist for the use of LLLT for musculoskeletal pain, osteoarthritis, muscle trigger points, tissue repair and muscle fatigue. During treatment of the tissue with the 635nm laser, an interaction between cells and photons takes place: a photochemical reaction. Photons from the laser affect the tissue at the cellular level. The cold laser enters the tissue, alters cell membrane permeability, and at the cellular level is absorbed in the mitochondria. – increased vascular activity. The laser stimulates lymph and blood circulation, to allow the affected tissue to have the best possible circulation
– stimulate nerve function. Slow recovery of nerve function in damaged tissue can result in “dead” limb or numb areas. Laser will speed the process of nerve cell reconnection to bring the numb area back to life. Laser also increases the amplitude of action potentials to optimize muscle action.

How do I use low level laser therapy LLLT ?

Simplelow level laser therapy LLLT Treatment Technique Example shown here is for a THORlow level laser therapy LLLT system with 200mW laser probe and cluster probe.

Having thoroughly palpated and assessed the area, set the timer to 20 seconds
Set the Pulse frequency to one of the following settings:
2.5 Hz – for acute injuries;
5 KHz – for chronic injuries and non-healing or infected wounds.
Treat the top of the neck/occiput atlas for 20 seconds
Then treat the nerve exits at C7 / T1 for 20 seconds
Then treat the nerve root exit related to injury for 20 seconds, and continue to treat several points along the course of the nerve towards the injury at 20 second intervals.
Treat each tender point for 20 seconds with the THOR 200mW laser probe. Palpate for any changes (eg: reduction in pain, change in tissue texture, relaxation of muscle, etc). If there is no response, then treat the tender points again. Palpate for any changes and repeat once more if necessary.
Finally treat the surrounding soft tissue with the cluster probe for 2 mins per area

Times are approximate and can be varied according to the dept of the injury / build of the patient.

Be accurate during treatment, gapping and positioning joints appropriately to gain maximum exposure of the joint, treating ‘around’ tendons where possible and be as thorough as time permits. Uselow level laser therapy LLLT as soon as possible after injury, treating daily for acute conditions and less frequently as resolution occurs. Treat chronic inflammatory conditions twice weekly and osteo-arthritis weekly or fortnightly. Treat as many points as necessary during one session; do not move the probe during each application. Treat with single, then cluster probe during one session.

There are normally no adverse effects fromlow level laser therapy LLLT, however, patients occasionally experience mild discomfort / ache after treatment. This is due to a restimulation of the inflammatory phase and should settle down after 24 – 48 hours.

How many time low level laser therapy Treatments are Needed?

The frequency and number of treatments depend on the conditions being treated.

Each individual will respond differently as low level laser works on the cells processes for the effect. Low level laser is suitable immediately before and after surgical procedures.

How much laser is enough? and how much is too much ?

In a recent systematic review and meta analysis of laser therapy on tendinopathies (Tumilty et al), 11 out of 20 studies failed to produce a positive result. The reason identified for the ineffective studies were that the laser beams were too strong (the irradiance was too high for this particular pathology). Most clinicians are rightly confused about which laser to choose and which dose or technique will work.

How to Find Low-Level Laser Therapy

Many massage therapists, acupuncturists, chiropractors, physical therapists, and other health care professionals use LLLT in their healing practices, and it may be easiest to find them in your area by asking your physician for a referral or searching online. Many companies now manufacture and sell low-level lasers to the general public; however, quality lasers can be quite pricey, ranging up to $25,000 or more, and there are many complex options to choose from. To provide an alternative to purchasing a laser for home use, some therapists rent their lasers to clients.

What are the contra-indications?
Eyes

THOR lasers have divergent beams but are potentially harmful if viewed directly from a distance of less than 1.1 meters. Patient, practitioner and observers should were THOR laser safety glasses when the laser is in use.

THOR LEDs are harmless and may even be helpful for some eye diseases when treated through closed eyes.
Relative contraindications / precautions
Cancer

Do not use LLLT over any known malignant lesions unless:

for pain relief during the terminal stages of the illness, but only with their physicians permission.
for cancer therapy side effects (e.g. oral mucositis, radiation dermatitis, etc), but only with their physicians permission.

Pregnancy

There is no evidence of harm to a an unborn baby however there are no safety tests either, so for medico legal reasons we recommend not treating directly over the developing foetus. It may be used on the pregnant woman for the treatment of back pain etc.
Thyroid

There is no evidence of harm and there is some evidence of benefit for treating hashimoto’s thyroiditis with LLLT. As with many things in life too much of what is good for us may be be harmful and so it is conceivable that a high intensity laser treatment direct to the thyroid might (temporarily) stimulate (or inhibit) some thyroid activity. We suggest not applying lasers directly over the thyroid. The THOR LED treatments are however relatively low intensity and far less likely to trigger any adverse events when treating that region of the neck.
Tattoo

Treatment over a tattoo with higher irradiance laser may cause pain as the dye absorbs the laser energy and gets hot. If treatment is painful remove treatment probe from contact and treat ~15mm from surface of skin.
Hair on the head

Treatment on the head and neck with high irradiance laser may cause pain as the melanin in the fine superficial hair follicle absorbs a lot of the laser energy. If treatment is painful remove treatment probe from contact and treat ~15mm from surface of skin.
Very dark skin

Occasionally some people with very dark skin feel an unpleasant amount of heating. If treatment is painful remove treatment probe from contact and treat ~15mm from surface of skin.
